Department of Transportation (DOT) Physical Exam for the Commercial Driver License (CDL)

The commercial driver license (CDL) physical exam is a requirement for all commercial drivers based on the Department of Transportation (DOT) rules.

People who drive commercial motor vehicles (CMV) are subject to physical qualifications. Unless excepted (exempted) by Federal or State law, these physical qualifications require a medical examination. The physical examination is required to help ensure that a person is medically qualified to safely operate a CMV.

What to expect during the Exam

The DOT physical exam process starts with the Medical Examination Report (MER) form. To cover the basic physical background of the driver, they to complete pages 1 and 2 on his or her own. The driver completes the form and brings it with them when they arrive for the physical exam.


A DOT medical exam involves a doctor's evaluation of a driver’s physical condition, vision, blood pressure, and a complete review of their medical history. This can help ensure that a driver does not have certain health conditions and can safely operate a commercial vehicle. Additionally, the exam includes urinalysis test and completing a medical paperwork. 
 

Preparing for Your Exam

To prepare for the exam, compile your most current medical records obtained from your primary care provider which should include all medical diagnoses & treatments of any available medical conditions. 

Right before your exam: 

  • Drink a glass of water and come in with a full bladder. 

  • Get adequate sleep

  • Do not consume energy drinks

  • Do not smoke/chew tobacco within one hour of exam

  • Do not eat or drink high sugar foods within 3 hours of exam

  • Take high blood pressure and diabetes medications as scheduled, if needed.

What to Bring
  • Photo ID

  • Medical records

  • List of current medications and dosage

  • Eye glasses or contacts worn when driving

  • Hearing aids

  • Waiver or exemption certificate documents 

  • Copy of your most recent stress test or medical release (for heart conditions, major medical issues)

  • Copy of your latest sleep test result (for sleep apnea)

Cost of Exam

 

The cost of the DOT/CDL Exam is $115.00 per applicant.

The fee is due at the time of service.
The fee includes the examination and completion of the required documents.


​Please note: This fee is not covered by medical insurance. 

Based on your medical records, additional tests may be required, which are not included in our Exam.
