USCIS Immigration Medical Examination

When applying for a Visa, Green Card, United States Citizenship, or change of status, an immigration medical exam is required to be completed.
BG Medical Center offers the Immigration Physical Exam at our office that’s conveniently located in Buffalo Grove, IL. We have many years of experience in performing immigration medical/physical examinations and completing the USCIS I-693 form paperwork (Report of Medical Examination and Vaccination Record).
Attention Ukrainian Citizens: BG Medical center is also providing all medical services that are listed under “medical conditions for humanitarian parole” issued by U.S. Customs and Border Protection for citizens of Ukraine who came to USA.

What to expect during the Exam
During your visit, the doctor will conduct a thorough immigration physical exam, including blood and urine tests.
The medical examination entails a review of your medical records.
The doctor will test for communicable diseases such as tuberculosis, syphilis, and gonorrhea.
After the exam, the doctor will complete Form I-693 and seal the form in an envelope for you to submit to USCIS.

What to Bring

A valid Passport.

Medical insurance card, if available

Your medical records for any chronic or serious medical problem. If medical records are not in English please have them translated before presenting for a medical visit.

Your immunization records.

Alien Registration number if you have one (starts with an “A”).

Payment for the exam

Form I-693 ("Report of Medical Examination and Vaccination Record "). We can print it out for you if needed.

Cost of Exam
The cost of the US Immigration Exam is $349.00 per adult or child applicant.
The fee is due at the time of service.
The fee includes the examination and completion of the required documents.
Please note: This fee is not covered by medical insurance.
Based on your medical records, additional tests may be required, which are not included in our Exam.
